Premium audit letter checklist and response-date countdown

Got a premium audit letter from your insurance carrier? Enter the response date and what the letter asks for. You get a day count to that date, a records checklist matched to each request, and two free CSV templates. Your letter sets the deadline, so ask your carrier about any extension.

We organize payroll records, track subcontractor certificates and can attend your premium audit with you where your carrier allows it. We do not sell or advise on insurance coverage. Your carrier assigns class codes. We do not represent you to the carrier.

What does the letter ask for? Tick every item it lists.

Enter the response-by date from your letter to see how many days are left.

Records to gather

A workers' compensation audit is based on payroll, so the payroll registers come first.

Start here

  • The audit letter itself, with the response date and the auditor's name and contact detailsEverything below is matched to this letter.
  • Policy number and policy periodEvery record you send should fall within these dates.
  • A note of each call or email with the carrier: date, name and what was agreedA written trail helps settle any later disagreement about what was said.

Your letter is the authority on what your carrier wants. If it lists something that is not here, add it to your own list and ask the auditor what format they accept.

Two free templates

Spreadsheet files (CSV). They open in Excel or Google Sheets.

What you enter is worked out in your browser. We record only that the tool was used, and we see your choices only if you submit the form.

Questions

Premium audit letter checklist and response-date countdown questions

What should I do first when I get a premium audit letter?

Find the response date and the list of records in the letter, and note the auditor's name and contact details. Then contact the carrier to confirm you received it and how they want records sent. Start with your payroll registers, because the auditor checks other records against them. The workers' comp premium audit checklist walks through each record.

What records do carriers usually ask for in a premium audit?

Letters differ, and yours governs. Carrier guidance commonly lists payroll registers, Forms 941 and state unemployment wage reports, 1099s and payments to subcontractors, subcontractor certificates of insurance, and sometimes the general ledger or tax returns. For the subcontractor side, see subcontractors and certificates of insurance in a premium audit.

Can I get more time to respond to a premium audit letter?

Only your carrier can move the date in your letter. Contact the carrier before the date passes, ask for the extension in writing, and keep a note of who you spoke with and when. The carrier decides whether to agree and for how long. If the date has already passed, call the carrier the same day.

Who can sit in on the premium audit with me?

Your carrier decides how the audit is run and who may take part, so ask when you confirm the audit. We organize payroll records, track subcontractor certificates and can attend your premium audit with you where your carrier allows it. See premium audit support for what that covers. We do not represent you to the carrier.
