Internal audit and risk

Internal audit process reviews for procure-to-pay, order-to-cash and payroll

Short answer

Finbryn provides internal audit process reviews of procure-to-pay, order-to-cash and payroll for CFOs and controllers of private companies and nonprofits, testing each cycle against its controls with samples and evidence and reporting findings to management and the audit committee. The AFP's 2025 survey, released April 15, 2025, found 79% of organizations faced payments fraud in 2024.

Reviewer's note

Software is up on last month after two seats were added mid-month. Revenue includes one milestone invoice that will not repeat next month.

Illustrative. An example of the document, not a client's figures.

Documenting a control and testing it are different jobs. Internal controls documentation writes down who approves a payment and who reconciles the bank account. A process review checks whether that happened, for a sample of real transactions, and says what it found. This page covers the testing.

A review starts from a cycle, not a department. Procure-to-pay runs from setting up a vendor to cash leaving the bank. Order-to-cash runs from a customer order to cash applied against the invoice. Payroll runs from a new hire record to the tax deposit. For each cycle Finbryn walks the process with the people who run it, selects samples from the full population, traces each one to its evidence and writes up every exception with the criteria, the condition found, the cause and the effect.

Reviews are sequenced by the risk-based internal audit plan. For many companies procure-to-pay comes first. The AFP found business email compromise was the top route for payments fraud in 2024, cited by 63% of respondents, and the ACFE found billing schemes in 424 of 1,921 cases. A vendor master file with one changed bank account is a more likely first finding than a missing policy.

The output is a findings report for management and the audit committee, with a response and a due date against every finding. Finbryn works from exports and read-only access that you grant, and the workpapers stay in your environment. Your staff answer questions, pull documents and respond to findings, and nobody is asked to change a process while it is being tested. A process review is not a fraud investigation and not an opinion on your financial statements.

What does a procure-to-pay internal audit test?

The cycle is tested control by control. Each row below is a control the review looks for, the evidence it asks to see and the exception that usually turns up.

StepControl testedEvidence examinedException that usually turns up
Vendor setupA second person approves each new vendor and a W-9 is on fileVendor request, approval log, W-9 recordVendors added and paid by the same user

Payment releaseThe person who approves a run is not the person who releases itBank user rights and approval logsOne user with both rights
ReconciliationA reviewer signs the monthly bank reconciliationSigned reconciliation and aged reconciling itemsOld items cleared without support

The full population goes through data tests first: duplicate amounts, round-dollar payments, payments just under an approval limit and vendors sharing a bank account or address with an employee. Samples are then drawn from what those tests and the risk assessment point to.

What does an order-to-cash review cover?

Order-to-cash controls protect revenue and the cash that follows it. The review tests that a customer is approved and credit-checked before a sale, that prices and discounts follow an authority table, that revenue is recorded in the right period, that credit memos and write-offs are approved by someone other than the person who issued the invoice, and that cash received is applied to the right invoice.

The tests lean on cutoff and exceptions. A sample of shipments and invoices straddling each period end shows whether revenue landed in the right month. A listing of credit memos by user and approver shows who can issue and who can approve. A comparison of the bank deposits to the cash application log shows how long cash sits unapplied. Cash application practices matter here because unapplied cash hides both errors and theft.

For nonprofits the same cycle covers pledges, grants receivable and restricted gifts, and the review checks that restrictions are recorded when the gift is recorded, not when the money is spent.

What does a payroll review cover?

Payroll is a closed loop, which makes it easy to test and easy to neglect. The review traces a sample of employees from the HR record to the pay register to the bank, and compares the register to the tax deposits and filings. It tests that new hires and terminations are approved and dated correctly, that pay-rate changes carry authority, that hours are approved before they are paid, that off-cycle checks are logged and that nobody on the payroll file lacks a matching HR record.

The ACFE's 2024 study counted payroll schemes in 190 of 1,921 cases, a median loss of $50,000, and found that payroll, billing and expense reimbursement schemes typically ran 18 months before detection. A single review a year will not catch everything, but it shortens the window in which an unapproved rate change or a former employee still on the register goes unseen.

Payroll processors such as Gusto or ADP run the payroll and own its calculations. Finbryn reviews the controls around the file and the approvals, using exports, and does not touch the processor's settings.

How does the review look for fraud exposure as well as control gaps?

A control gap is a place where something could go wrong. Fraud exposure is a place where someone has the access and the incentive to make it go wrong. The review reads both. The ACFE's 2024 report counted billing schemes in 424 of 1,921 cases (22%) with a median loss of $100,000. Check and payment tampering appeared in 217 cases (11%) with the highest median loss of the asset misappropriation schemes, $155,000. Expense reimbursement schemes appeared in 248 cases with a median loss of $50,000.

The AFP's 2025 survey adds the outside view: 79% of organizations faced attempted or actual payments fraud in 2024, business email compromise was the top route at 63% and vendor impersonation was cited by 45%. That is why the vendor bank-detail change process is tested before almost anything else.

When a test produces a pattern that looks like fraud, the review stops at the facts, tells the audit committee and recommends a specialist. It does not interview suspects or build a case. Forensic accounting support is where that work starts, and the committee decides whether to take it up.

What is in the findings report?

Every finding has the same shape so the committee can compare them: the criteria (what should happen), the condition (what did happen), the cause, the effect, the recommendation, a management response with a named owner and a due date, and a rating on a scale the committee defined before the first review. Positive observations are listed too, because a report that only lists problems teaches management to argue with it.

The report goes to the process owner for a written response before it goes to the audit committee, and the committee receives the finding and the response together. Standard 15.2 of the Global Internal Audit Standards has the chief audit executive confirm that agreed actions were implemented, so the follow-up log stays open until each action closes, and the committee sees the log at each meeting.

The workpapers are yours. They carry the population, the sample, the evidence reference and the result for each item, and they are stored in your environment so a successor, a lender or your external auditor can read them.

What does a process review not do?

Internal audit support here is planned and reported against the IIA Global Internal Audit Standards for management and the audit committee. Conformance is the chief audit executive's to demonstrate through the function's quality program. It is not an external audit, review, compilation or attestation, and Finbryn gives no opinion on financial statements or on internal control over financial reporting under AICPA or PCAOB standards.

It also does not conclude on a fraud allegation, test controls at a service organization such as a payroll processor, or replace the controls the company has to run itself. Sample-based testing can miss an exception that is not in the sample, and the report says what the sample was. If a lender requires an examination report on internal control, an independent public accounting firm issues it.

How we work

The process

  1. 1

    Agree the objective and scope

The cycle, the period, the systems and the exports we need are written into a scoping memo that the audit committee has seen.

  2. 2

    Walk the process

We follow one transaction end to end with the people who run it and confirm the narrative with them.

  3. 3

Run data tests on the full population

Duplicates, round amounts, just-under-limit payments and shared bank details are tested before any sample is drawn.

  4. 4

    Test the sample

Each item is traced to its evidence. Exceptions are logged with the facts as found.

  5. 5

    Draft findings and obtain responses

Owners respond in writing to each finding, with an action and a date.

  6. 6

    Report and follow up

The audit committee receives findings and responses together, and the follow-up log stays open until each action closes.

Common problems we fix

  • A vendor's bank details were changed from an email and nobody can show who approved it
The review tests every bank detail change in the period against a call-back record and flags the ones without one.
  • The same user can create a vendor, approve the invoice and release the payment
User rights are exported and compared to the approval log, and the report names the combinations that break segregation of duties.
  • Credit memos and write-offs are approved by the person who issued them
A listing by user and approver shows the overlap, and the recommendation sets an approval limit with a second approver above it.
  • Payroll has an employee with no matching HR record
The register is compared to the HR file and the tax deposits, and every unmatched line is traced to its authorization.

Questions

Frequently asked questions: Internal audit process reviews for procure-to-pay, order-to-cash and payroll

What does a procure-to-pay internal audit test?

Vendor setup and approval, changes to vendor bank details, matching of order, receipt and invoice, duplicate payments, who can release a payment run and whether bank reconciliations are reviewed. The ACFE's 2024 report found billing schemes in 424 of 1,921 cases and the AFP's 2025 survey, released April 15, 2025, found business email compromise was the top payments fraud route, so these are the first controls tested.

How do I audit vendor payments for fraud?

Run data tests on the whole payment file before sampling: duplicate amounts and invoice numbers, round-dollar payments, payments just under an approval limit and vendors sharing a bank account or address with an employee. Then trace a sample to approvals and bank-change records. The AFP's 2025 survey (April 15, 2025) put vendor impersonation at 45% of respondents.

What is the difference between documenting controls and testing them?

Documenting records how a process is supposed to run: who approves, who reconciles and how often. Testing checks a sample of real transactions against that description and reports where practice differs. COSO's 2013 Internal Control framework, with five components and 17 principles, is the usual yardstick for both (checked 2026-10-16).

How big is the sample?

It depends on the population, the risk and how often the control runs, and the workpaper states the method. A control that runs daily gets a larger sample than one that runs once a quarter. The data tests cover the whole population, so the sample is drawn from where those tests and the risk assessment point. In the ACFE's 2024 study, billing, payroll and check tampering schemes typically ran 18 months before detection, which is why the data tests span the full period.

Will a review find all the fraud?

No. Sampling can miss an item that is not in the sample, and the ACFE notes that a control being present does not mean fraud cannot occur. In its 2024 study of 1,921 cases, billing, payroll and check tampering schemes typically ran 18 months before detection. A review shortens that window and shows where to look next.

What access do you need?

Exports and read-only access to the accounting system, the payment platform and the HR or payroll file, granted by you. We do not ask for the ability to post, approve or release payments. Access is listed in the scoping memo and removed when the engagement ends (checked 2026-10-16).

Can the same team that keeps our books review the payables cycle?

No. Under Finbryn's independence policy we do not perform internal audit work on a process we run for you or ran in the previous 12 months. If Finbryn runs your payables, your own staff or another provider tests that cycle, or it stays out of the plan (Global Internal Audit Standards, effective January 9, 2025).
